For operators of nuclear research facilities, it is of particular importance to investigate minor incidents: indeed, as safety demonstrations are generally based on the presence of several independent "lines of defence", only through attentive investigation of every occurrence, usually minor and of no consequence, can the level of trust placed in each of these defensive lines be confirmed, or the potential risks arising out of a possible weakness in the system be anticipated. The efficiency of the system is based on a rigorous procedure: stringent attention to all incidents, consideration of the potential consequences of the incidents in their most pessimistic scenarios, and promotion of a broad conception of transpositions of the events, in time and space, for experience feedback. This efficiency presumes motivation on the part of all those involved, hence the importance of dissociating from the concept of an "incident" any notion of "error" or "blame" both in internal analysis and in public communications. The nuclear industry has developed some very progressive tools for experience feedback, which could interest also management of other technological risks. This book presents the proceedings of a NATO Advanced Workshop dedicated to this important matter of concern.

The two-volume course "Greek to GCSE" was written in response to a JACT survey of over 100 schools and first published in 2003. The course offer a fast-track route to GCSE for those with limited time. It is based on experience of what pupils find difficult. It concentrates on the essentials, and on the understanding of principles in both accidence and syntax. It aims to be user-friendly, but also to give pupils a firm foundation for further study. Part 1 covered the basics: the main declensions, a range of active tenses, and a vocabulary of 275 Greek words to be learned. Part 2, here issued in a new edition, introduces a wider range of grammatical forms and constructions. This revised Safety Improvements Through Lessons Learned from Operational Experience in Nuclear Research Facilities pdf edition has been adapted to complete the coverage of requirements for the current GCSE, expanding vocabulary to 480 words. Inspired by one of the world s greatest novels, Ohio artist Matt Kish set out on an epic voyage of his own one day in August 2009. More than one hundred and fifty years following the original publication of Moby-Dick, Kish began illustrating Herman Melville s classic, creating images based on text selected from every page of the 552-page Signet Classics paperback edition. Completely self-taught, Kish refused to set any boundaries for the artwork and employed a deliberately low-tech approach in response to the increasing popularity of born-digital art and literature. He used found pages torn from old, discarded books, as well as a variety of mediums, including ballpoint pen, marker, paint, crayon, ink, and watercolor. By layering images on top of existing words and images, Kish has crafted a visual masterpiece that echoes the layers of meaning in Melville s narrative. In retrospect, Kish says he feels as foolhardy as Ishmael, the novel s narrator, and as obsessed as Captain Ahab in his quest for the great white whale. I see now that the project was an attempt to fully understand this magnificent novel, to walk through every sun-drenched word, to lift up all the hatches and open all the barrels, to smell, taste, hear, and see every seabird, every shark, every sailor, every harpooner, and every whale, he says. It was a hard thing, a very painful thing, but the novel now lives inside me in a away it never could have before. Kish spent nearly every day for eighteen months toiling away in a small closet he converted into an art studio.
